Case 4: Chronic Synovitis Secondary to Gout

Patient SAS is a 64-year-old man with a 14-year history of gout. At age 51, he presented with acute unilateral gout in the first metatarsophalangeal joint with recurrent attacks every 2 to 3 years. These attacks were responsive to col-chicine. At this time, his serum uric acid level was 8.3 mg/dL (0.49 mmol/L). Upon questioning, he stated that he was a social drinker of alcoholic beverages, with increased weekend consumption of distilled spirits
