At 50 years of age, this patient presented with progressively worsening diplopia. Her work-up at that time demonstrated a one and a half centimeter aneurysm arising from the cavernous segment of the internal carotid artery and a small incidental left middle cerebral artery aneurysm. During her initial evaluation at an outside hospital, the patient failed a balloon test occlusion performed to determine her ability to tolerate therapeutic carotid occlusion. She subsequently underwent elective clip ligation of the middle cerebral artery aneurysm and no specific treatment was provided for her cavernous carotid aneurysm.
