The authors deserve to be commended for the minimally invasive open surgical transvesical prostatectomy described. To most urologists, who perform just a few open prostatectomies yearly, the procedure described, with acceptable blood loss despite not suturing the individual bladder neck arteries seems too good to be true and also scary. However, the results speak for themselves. That all 165 procedures were performed by a single surgeon is proof that practice and experience can defy conventionally laid down norms and ground rules while practicing the art of surgery. The technique used in their study reminds us of an accepted practice decades ago when preliminary open or trocar suprapubic cystostomy used to be done and followed up weeks later by sutureless enu-cleation through the cystostomy tract using finger dilation of the tract.
