Re: Kim et al: influence of Prostatic Calculi on Lower Urinary Tract Symptoms in EVEiddle-aged Men (Urology 2011;78:447-449)

We read with much interest the published report "Influence of Prostatic Calculi on Lower Urinary Tract Symptoms in Middle-Aged Men" by Kim et al. The authors meticulously discuss the relationship of prostatic calculi with lower urinary tract symptoms. The authors concluded that large prostatic calculi are a significant associated factor of moderate lower urinary tract symptoms with a 1.784-fold increase in risk compared with no and small calculi. We wish to discuss some important facts related to the topic.It would have been interesting if an analysis of the International Prostate Symptom Score had been performed (ie, whether the symptoms affected were more in terms of storage, such as nocturia, frequency, urgency, or more in terms of voiding, such as hesitancy, straining, terminal dribbling, poor stream). Coupled with the uro-flow assessment, it would have shown whether the symptoms were more obstructive or irritative in nature, because that would influence which type of treatment would be beneficial. Storage symptoms would correlate more with a prostatitis pathogenesis, and voiding symptoms more toward aging and prostate enlargement. We believe an analysis comparing the prostate volume and calculi type (ie, small, large, or none) would have been more useful.
