Are the anterior internal capsules and nucleus accumbens suitable DBS targets for the treatment of depression?

Chang et al. (Chang et al., 2009) report the case of a 28 year old male treated with bilateral deep brain stimulation (DBS) of the anterior limbs of the internal capsules for refractory obsessive-compulsive disorder (OCD) of 10-year duration. At the patient's initial presentation also a diagnosis of bipolar 1 disorder was made. The authors observed two weeks after intervention that stimulation through the ventral contact 1 at 4 volts resulted in a decrease in the obsessive thought while causing hypomanic symptoms (talkativeness, grandiosity, euphoria, and flight of ideas). Turning off the stimulation resolved immediately the hypomanic event.
