Human error and a human tragedy

The American Airlines flight from Miami to Cali, Colombia, five days before Christmas should have been uneventful. Under CEO Robert Crandall, the airline had built one of the best safety records in the business. The type of plane winging the mostly Colombian passengers home for the holidays, a Boeing 757, has a nearly flawless safety performance, and so had the pilot and co-pilot. And the weather was clear.
