A report by the Rail Accident Investigation Branch (RAIB) into a fatal accident on a foot crossing in Nottingham, where an elderly woman and young child were killed in November 2008, says the risks from the adjacent tram line were never considered by either Network Rail or the tram operators, Nottingham Express Transit.
