The Raib report into the derailment of the tender of'Flying Pig' 4MT 2-6-0 No. 43106 at Hampton Loade on September 28, 2009, identifies two causal factors.rn1) A tender spring had been replaced by a spring of an incorrect type. 2) The track had become distorted (a twist had developed) without track maintenance staff being aware of the problem.rnRaib makes five recommendations concerning the Severn Valley Railway's safety management systems, engineering procedures, audit processes and communication of safety information.
