WHENEVER there is a serious incident on public transport resulting in injuries or fatalities, the phrase 'safety is our No. 1 priority' is always trotted out by a spokesperson. The RAIB report on the two track workers killed at Margam makes disturbing reading, particularly in relation to the prevailing safety culture and a lack of willingness to challenge working practices.
