One vital component in the evolution of safer civil aviation is its ability to learn from past accidents and incidents. Over the decades many design and procedural changes have come about as a result of crashes, near-crashes and incidents; but it seems that three tragedies from the past - and the lessons learned - may have been forgotten now that they have faded from the headlines. G-ALAM Left Sydney at about 2130 local time to return to London. There were two pilots, two flight engineers, a radio operator, navigator, three cabin crew and 31 passengers on this four-engine piston aircraft. They flew Sydney - Darwin - Djakarta -Singapore, a total of about 4000 nm; the three sectors would have taken at least 16 flying hours. As they approached Singapore for a tight, visual approach onto runway 06 they had been on duty (according to the accident report) for at least 21 Vb hours, and it was now the following afternoon local time.
