One vital component in the evolution of safer civil aviation is its ability to learn from past accidents and incidents. Over the decades many design and procedural changes have come about as a result of crashes, near-crashes and incidents; but it seems that three tragedies from the past - and the lessons learned - may have been forgotten now that they have faded from the headlines.
