The term non-puerperal mastitis encompasses a heterogeneous spectrum of different inflammatory changes of the female breast. Bacterial infections should be treated with broad spectrum antibiotics. Abscesses should undergo punctation or drainage. Recurrent infections need a special surgical technique. The fibroadenoma is the most frequent benign breast tumor of the female. It needs to be histologically confirmed but does not really need to be excised. Hamartomas are tumors which mirrors all structures of the breast. They do not exhibit malignant potential. Adenomas can not be distinguished from fibroadenomas by imaging techniques. Evidence of lipomas comes from clinical examination, mammography, and ultrasonography. They should be removed surgically in order to exclude the possibility of a liposarcoma. Serous nipple discharge is suspicious of papillomas of the major ducts. These should be removed from the duct. A cystosarcoma phylloides grows rapidly and must be removed surgically with clear margins. Recurrences are not uncommon, partly with malignant transformation. Small, fat tissue necrosis is common in a large breast, especially after breast conserving therapy for breast cancer. Surgical treatment is only rarely necessary.
