The 11th Population Health and Care Coordination Colloquium was held March 14-16, 2011 in Philadelphia, Pennsylvania. Colocated with the Medical Home Summit and the Palliative Care Summit, the Colloquium assembles a diverse group of professionals who are working to transform the health care system.
