1. What were the challenges to the accuracy of the sensors within containment and in the fuel pool during Fukushima and TMI? The events at Fukushima have resulted in a heightened awareness in the nuclear industry about issues with existing instrumentation that cannot adequately inform plant operators of the reactor parameters in severe accident conditions. For example, determination of in-core coolant level and temperatures at TMI and Fukushima were challenging to plant operators due to lack of in-core level instrumentation, insufficiency of instrumentation in the blackout conditions (Fukushima), and faulty in-core temperature measurements in the severe conditions (TMI).
