The late US president, Lyndon Johnson, once said: 'Show me a man who never made a mistake, and I will show you a man who never did anything.' In today's 'I-told-you-so' society, where it seems there is always a body of opinion only too ready to criticise individuals and organisations for daring to try something new, the fear of being blamed for making a mistake is one of the biggest barriers to change and innovation.
