PRISE EN CHARGE ET TRAITEMENT DE L'ACCIDENT VASCULAIRE CÉRÉBRAL

Four main points in the management of stroke during the pre-hospitalization phase have to be considered: indication of thrombolysis (duration of the symptoms less than 3 hours), importance of a quick management in case of TIA which constitutes an important risk factor for a definite stroke and detection of an etiologic factor (cardiovascular risk factors, carotid stenosis, embolic cardiopathy) which may guide the therapeutic choice. Furthermore administration as soon as possible of an antiplatelet agent (aspirin 250 mg) improves the outcome (diminishes the mortality) and reduces the rate of stroke recurrence. This treatment should therefore be started at the patient's home.%Quatre points essentiels doivent être considérés lors de la prise en charge d'un AVC dans la phase pré-hospitalière : l'indication à une thrombolyse (délai d'installation des symptômes de moins de trois heures), la nécessité d'une prise en charge rapide en cas d'accident ischémique transitoire, qui constitue un facteur de risque important pour le développement d'un infarctus irréversible, et la recherche d'un facteur étiologique (FRCV, sténose carotidienne, cardiopathie emboligène) pouvant orienter le choix du traitement ; enfin, l'administration aussi rapide que possible d'un traitement antiagrégant plaquettaire (aspirine 250 mg) améliore le pronostic (diminution de la mortalité) et réduit le taux de récidives. Ce traitement devrait par conséquent être débuté au domicile du patient.
