Anemia is a frequent condition in oncologic patients and influences patient survival rate. A good hemoglobin level is related to better patient conditions and improves the outcome of therapies. Authors report the possible mechanisms inducing anemia in bone oncology, as histotype, chemotherapy and radiotherapy, surgery, nutrition, discuss the problems transfusion related, and report the options of blood saving in the different times: before surgery, intraoperatively and postoperatively. In particular attention should be given to the preoperative administration of erythropoietin, to less invasive surgical technique for reducing transfusion and improving hemoglobin level.
