Introduction

Economists have written countless papers analyzing behavior in the American Medicare program, including the justly famous Dartmouth work on geographic variation (e.g., Skinner, 2012), but the vast majority of those papers have analyzed behavior in the original "fee-for-service" Parts A and B of the program, which we term traditional Medicare. In recent years, however, Parts C and D of the program have gained prominence, both of which rely more on standard market mechanisms than does traditional Medicare.
