Duration of Initiative: Three months to develop, now ongoing Setting: Denver Health (DH) is a public health safety-net integrated delivery system including nine community health centers, one hospital, 12 school-based clinics, a Level-I trauma center, a poison center, and the Department of Public Health. DH had approximately 20,000 inpatient admissions and more than 600,000 outpatient visits during 2004.
