Medication Errors: Experience of the United States Pharmacopeia (USP)

The United States Pharmacopeia (USP) is a practitioner-based organization that sets standards for the identity, strength, quality, purity, packaging, labeling, and storage of therapeutic products. USP's standards-setting body is the Council of Experts. This council maintains and continuously revises the United States Pharmacopeia and National Formulary (USP-NF). As a nonprofit organization working in the public interest, USP also operates several public health programs seeking to further ensure that practitioners, patients, and consumers have access to high-quality therapeutic products and that they use these products safely. Patient safety is one of these programs.
