The Marine Department concludes, "The investigation has established that the main causes of the accident were the failure of the Master and Chief Officer of Santa Maria to observe Rule 5 of COLREGS to maintain a proper lookout, and the failure of the Master of Funchal to observe Rule 8 of COLREGS by not reducing the speed of the vessel to allow more time to assess the situation."
