Canadian Urological Association best practice report: Holmium:YAG laser eye safety

Since the introduction of the Holmium:yttrium-aluminumgarnet(Ho:YAG) laser over two decades ago, it has becomean indispensable tool in the urologist’s armamentarium.More specifically, in the subspecialty of endourology, theHo:YAG laser has revolutionized the approach to kidney,ureteral, and bladder stones, endoluminal tumors, strictures,and benign prostatic hyperplasia (BPH).1 Since its widespreadadoption, and as with other laser wavelengths, therehave been concerns regarding its safety. In particular, thepotential risks of eye injury to patients and operating room(OR) personnel have led laser manufacturers and governingbodies (including the Canadian Standards Association [CSA])to require/mandate that all intraoperative personnel wearlaser safety goggles. Despite this recommendation, the actualrisk of eye damage associated with the Ho:YAG laser duringendourological procedures has not been clearly defined.The objectives of this best practice report are to review thecurrent literature regarding the risks of eye injuries and toprovide practical and evidence-based recommendations oneye safety with the use of the Ho:YAG laser. This best practicereport was developed in conjunction with members of theCanadian Endourology Group.
