Hospitals, clinics and pharmacies shouldabide by the five rights of medicationuse – the right drug to the right patientin the right dose by the right routeat the right time. Yet in all stages of amedication process, that is, prescribing,transcribing, dispensing, administeringand monitoring, medication errors andadverse drug effects still take place.
