To the Editor: Virus-associatedhemophagocytic syndrome (VAHS) isa fulminant disorder associated withsystemic viral infection and is charac-terized pathologically by the prolifer-ation of hemophagocytic histiocytesin the lymphoreticular tissues. Herewe report a case of mumps VAHS fol-lowing parotitis and pancreatitis.A 39-year-old, previously healthywoman sought treatment for abdomi-nal pain on June 14, 2002. On physicalexamination, her bilateral parotidglands were swollen, and her leftupper quadrant was tender. Laboratorystudies showed a leukocyte count of4,640/mm3, a hemoglobin concentra-tion of 13.9 g/dL, and a platelet countof 19.1 × 104/mm3. The level of amy-lase was elevated in her blood (1,613IU/L; normal 50–160 IU/L) and urine(12,940 IU/L; normal 200–1,100IU/L). Her level of pancreaticenzymes was also elevated: lipaselevel was 194 IU/L (normal 7–60IU/L) and phospholipase A2 level was1,340 ng/dL (normal 130–400 ng/dL).Parotitis and acute pancreatitis due toa mumps virus infection were diag-nosed.
