Objective: To study the molecular mechanisms for Haikunshenxi capsule combined withconventional western medicine to improve renal function in patients with diabetic nephropathy.Methods: A total of 146 patients with diabetic nephropathy treated in our hospital betweenMay 2013 and December 2015 were selected as the research subjects and randomly dividedinto two groups, 73 in each group. The observation group of patients received Haikunshenxicapsule combined with conventional western medicine treatment, and the control groupof patients received routine western medicine treatment. 4 weeks after treatment, the renalbiochemical indexes, renal fibrosis indexes and oxidative stress indexes in blood and urinewere determined. Results: 4 weeks after treatment, serum creatinine (Scr), blood urea nitrogen(BUN), cystatin C (CysC), transforming growth factor-β1 (TGF-β1), connective tissue growthfactor (CTGF), fibronectin (FN), laminin (LN) and type IV collagen (IV-Col) content in bloodof observation group were significantly lower than those of control group (P0.05), and 24h urine protein, urine albumin/creatinine ratio (UACR) level as well as alkaline phosphatase(ALP), γ-glutamyl transpeptidase (GGT), malondialdehyde (MDA) and 8-hydroxy-2'-deoxyguanosine (8-OHdG) content in urine of observation group were significantly lowerthan those of control group (P0.05) while superoxide dismutase (SOD) and glutathioneperoxidase (GSH-Px) content in urine were significantly higher than those of control group(P0.05). Conclusions: Haikunshenxi capsule combined with conventional western medicinecan improve renal function in patients with diabetic nephropathy through the molecularmechanisms of inhibiting renal fibrosis and reducing oxidative stress.
机译:目的:探讨海昆参西胶囊联合中西医结合改善糖尿病肾病患者肾功能的分子机制。方法:选择2013年5月至2015年12月在我院治疗的146例糖尿病肾病患者作为研究对象。随机分为两组,每组73个。观察组采用海昆参西胶囊联合常规西药治疗,对照组为常规西药治疗。治疗4周后,测定血液和尿液中的肾生化指标,肾纤维化指标和氧化应激指标。结果:治疗后4周,血清肌酐(Scr),血尿素氮(BUN),胱抑素C(CysC),转化生长因子-β1(TGF-β1),结缔组织生长因子(CTGF),纤连蛋白(FN),层粘连蛋白观察组血液中LN和IV型胶原的含量均明显低于对照组(P <0.05),且24h尿蛋白,尿白蛋白/肌酐比值(UACR)和碱性磷酸酶水平均显着低于对照组。观察组尿液中ALP,γ-谷氨酰转肽酶(GGT),丙二醛(MDA)和8-羟基-2'-脱氧鸟苷(8-OHdG)的含量均明显低于对照组(P <0.05)尿液中的歧化酶(SOD)和谷胱甘肽过氧化物酶(GSH-Px)含量明显高于对照组(P <0.05)。结论:海昆参西胶囊联合常规西药可通过抑制肾纤维化,降低氧化应激的分子机制改善糖尿病肾病患者的肾功能。
