Association of serum fibrinogen with severity of acute coronary syndrome

Background: Cardiovascular disease is the leading cause of deathworldwide and a majority of these deaths are attributable toMyocardial infarction. Due paramountcy was given to evaluatethe correlation between the acute phase reactants and theseverity of acute coronary syndrome. One marker which wasunder great a deliberation for long was fibrinogen. Albeit thoroughresearch, the association was debated till recently when many ofthe western studies showed a correlation. None of the studieswere done on Indian population.Objective: To study the correlation between serum fibrinogenlevel (SFL) and severity of ACS.Methods: This is a prospective study on hospitalized patients withACS. ACS was diagnosed based on clinical presentation, ECG andcardiac biomarkers. Blood for fibrinogen was drawn at admissionand was measured by chemiluminescence technique. Coronaryangiography was performed through the femoral artery accessand the angiograms were evaluated by interventional cardiologistswho will be blinded to the study. The severity of ACS wasassessed by Gensini score. Data analysis and interpretation wasdone with SPSS 20. The statistical significance of association wasassessed by chi square test and p value of less than 0.05 wasconsidered significant.Results: We serially analysed 91 patients presenting with ACSfrom February 2014 to April 2014. 29 patients were excluded as perpre-specified criteria. SFL was plotted against Gensini score in theremaining 62 patients. Pearson抯 correlation coefficient betweenSFL and Gensini score was r?.869. We have found a significantcorrelation between SFL and Gensini score with a significant pvalue (<0.05).Conclusion: Statistically significant correlation exists between SFLand Gensini score. Higher SFL were associated with higher Gensiniscore. We hence conclude to say that serum fibrinogen can beused to assess the severity of ACS.
