A Tale of Two Emergency Visits

To the editor: A 70-year-old mandevelops aphasia and unilateralweakness. He is transported byambulance to a nearby academictertiary care hospital. A ‘‘strokecode’’ is initiated, and within 45minutes of the onset of symptoms,tissue plasminogen activator is running. Imaging reveals competeobstruction of his right middlecerebral artery, but a repeat scan24 hours later shows minimal residual ischemic changes. Over the next48 hours, his symptoms resolvecompletely. He has no functionaldeficits.
