Students of air safety investigation know that most accidents and incidents arise from a series of small events that slip unnoticed into the timeline rather than one catastrophic occurrence. Small mechanical stresses grow into large system failures; cockpit workload gradually increases until the crew falls behind the situation; and, sometimes, luck (good or bad) enters the equation and skews the outcome.
