NTSB investigators took a long look at the human factors elements in the May 15, 2017, loss of a Trans-Pacific Air Charter Gates Learjet 35A (N452DA) and its two crewmembers while attempting an ILS 6 circle to 1 approach in VMC at Teter-boro (NJ) Airport. The cockpit voice recorder was damaged by impact and fire, but technicians were able to pull out 30 min. of good audio. The second-in-command (SIC) in the right seat was the pilot flying (PF), and the captain in the left seat was the pilot monitoring (PM). He was also providing step-by-step instructions to the SIC.
