As now, no randomized controlled clinical trials exist identifying the best treatment of osteochondral lesions. Staging with X-ray and MRI imaging aids in implementing the therapeutic algorithm, increasing the rate of success. Arthroscopic microfracture is the most validated treatment for symptomatic lesions less than 15 mm2 in area, while open surgery with autologous or heterologous osteochondral transplantation is needed for the other lesions.
