显微技术下颈前入路治疗脊髓型颈椎病

摘要

Objective To evaluate the effect of microscope-guided anterior cervical discectomy with fusion(ACDF) in the treatment of cervical spondylotic myelopathy. Method The clinical data of 33 patients with cervical spondylotic myelopathy treated by anterior cervical discectomy with fusion(ACDF) in our hospital from 2014 till now were analyzed retrospectively. Japanese Orthopaedic Association(JOA) score and its improvement rate were used to evaluate neurologic function after operation. Results All patients were followed up for 3 months. JOA score improved significantly from preoperative 11.0 to postoperative 15.4(P<0.05). Conclusions Anterior cervical decompression and fusion by microscope is a refined,safe and effective surgical procedure for cervical spondylotic myelopathy.%目的 探讨颈前入路显微手术治疗脊髓型颈椎病的疗效.方法 回顾性分析2014年10月至2016年8月33例采用颈前路椎间盘切除减压融合术(ACDF)治疗脊髓型颈椎病患者的临床资料,以日本骨科学会(JOA)评分(17分法)及其改善率评价患者术前、术后3个月神经功能,评价手术效果.结果 患者术后神经功能均明显改善.术前平均JOA评分11.0分,术后3个月平均JOA评分15.4分,差异有统计学意义(P<0.05).术后平均改善率73.3％,无脊髓损伤、术后出血等严重并发症.结论 显微镜技术颈前入路减压融合术治疗脊髓型颈椎病,视野清晰,止血可靠,同时可恢复颈椎曲度和椎间隙高度,手术创伤小,临床疗效满意.
