目的 比较关节镜与传统开放手术清理膝关节痛风石的临床效果.方法 回顾性分析2010年8月至2015年9月膝关节痛风石患者43例,根据手术方式分为关节镜组(25例)和传统手术组(18例),随访6～18个月,比较两组Lysholm评分、住院时间、切口感染及末次随访复发率.结果 两组Lysholm评分术前差异无统计学意义(P>0.05),术后2周及末次随访关节镜组Lysholm评分高于传统手术组(P<0.05);住院时间关节镜组明显短于传统手术组(P<0.01);传统手术组发生切口感染2例,经清创后愈合,关节镜组无术后感染;末次随访两组患者均未见复发.结论 关节镜下清理膝关节痛风石手术创伤小,住院时间短,术后膝关节功能恢复优于传统开放手术.%Objective To compare the clinical effect between arthroscopic surgery and conventional surgery in the treatment of knee tophi. Methods A retrospective analysis was conducted,clinical data was collected from August 2010 to September 2015,a totally 43 patients with knee tophi were selected and divided into arthroscopic surgery group and conventional surgery group according to different surgical approaches. The Lysholm score,length of hospital stay,wound infection and recurrence rate were compared. Results 25 cases of arthroscopic surgery group,18 cases of conventional surgery group were enrolled in this study;no significant difference was found between groups of Lysholm score before surgery(P>0.05), nevertheless,Lysholm score of arthroscopic surgery group was higher than conventional surgery group at two week and last follow-up after surgery (P<0.05). Hospital stay was shorter in arthroscopic surgery group(P<0.01);two cases in conventional surgery group got wound infection and healing after debridement,no postoperative infection was found in arthroscopic surgery group;patients in both groups showed no recurrence at last follow-up. Conclusion Clean up the knee tophi under arthroscopy which has advantages of minimal trauma and shorter hospitalization time,is superior to the conventional surgery in the recovery of knee joint function.
