目的 比较三阴性乳腺癌(TNBC)和非TNBC的临床病理特征,并与复发特征进行相关性分析,为乳腺癌个体化治疗丰富理论依据.方法 统计2000年2月至2011年12月789例单侧乳腺浸润性导管癌患者,其中TNBC患者共98例,对其临床病理特征、复发时间及预后进行相关性分析.结果 在中位随访期101(3~200)个月的随访中,27例(27.6%)TNBC出现复发,与非TNBC组18.5%的复发率相比差异有统计学意义(P<0.05).其中位复发时间是20(2~115)个月,2年内发生复发的患者占63.0%,TNBC发生肺部转移的概率高于非TNBC,且更易发生局部复发(P<0.05),影响TNBC总生存的独立因素为临床分期.结论 TNBC和非TNBC相比存在独特的复发转移特征,预后相对更差,需要进一步丰富TNBC个体化治疗的研究.%Objective To compare the clinical pathological characteristics of triple-negative breast cancer(TNBC)and non- triple-negative breast cancer,and explore their recurrence characteristics,enrich evidences for individualized combined treatment for breast cancer. Methods Regression analyses were performed for 789 unilateral breast invasive ductal carcinoma patients from February 2000 to December 2011. Among them,there were 98 TNBC patients. The correlation of the clinical pathological characteristics,recurrence and prognosis were analyzed. Results The median follow-up period was 101(3-200)months. And 27(27.6%)TNBC patients recurred and it was significantly different from the recurrence rate of non- TNBC group(18.5%,P<0.05). The median time of recurrence was 20(2-115)months and 63.0% recurred within 2 years. The probability of lung metastasis was higher in TNBC patients than those in non-TNBC patients,and also had higher local recurrence rates(P<0.05). The independent prognostic factor of TNBC was clinical stage. Conclusion TNBC has unique characteristics and worse prognosis compared with TNBC. Further study is needed to find individualized treatment for TNBC.
