关节镜手术联合非布司他对膝关节痛风性关节炎诊治的价值

摘要

目的：探讨关节镜微创技术联合非布司他在急性膝关节痛风性关节炎诊治中的应用价值。方法35例膝关节痛风性关节炎患者行关节镜下关节腔清理术，术后应用非布司他长期口服控制尿酸。随访6~24个月。结果35例患者膝关节腔内均发现炎性滑膜增生明显，白垩状尿酸盐结晶沉积于滑膜、软骨、半月板及交叉韧带等处，部分患者关节内软组织结构破坏明显，并在关节镜下实施清理治疗。末次随访时Lysholm评分较术前明显增高（P<0.05），VAS评分较术前明显降低（P<0.05）。结论关节镜手术可以微创、彻底清除膝关节腔内沉积的尿酸盐结晶，迅速缓解症状，结合长期非布司他口服，可以达到减少复发，稳定病情的目的。%ObjectiveTo evaluate the efficacy of combination of arthroscopic surgery and Febuxostat in the treatment of gouty arthritis. Methods Thirty-five knees with gouty arthritis were treated with arthroscopic surgery and Febuxostat were used after the operation as long term therapy. All 35 cases were followed up for 6-24 monthes.Results 35 cases of patients were found infl ammatory synovial hyperplasia in knee joint cavity obviously,the Cretaceous uric acid salt crystals deposited in the synovial,cartilage,meniscus and cruciate ligament and other places,Some patients' joints were badly damaged. The Lysholm score was significantly higher than that before operation,VAS score was significantly lower(P<0.05). Conclusion Arthroscopic surgery can be minimally invasive,can complete removal of the knee joint cavity deposition of uric acid salt crystallization, rapid relief of symptoms. When combined with long-term Febuxostat treatment,can achieve the purpose of reducing the recurrence,stable condition.
