参黄散外敷对术后肠功能障碍大鼠结肠传输及血浆胃肠激素水平的影响

             

摘要

目的 探讨参黄散对术后肠动力障碍(POI)大鼠结肠传输及血浆胃肠激素水平的影响.方法 将24只SD雄性大鼠随机分为假手术组、模型组和参黄散组,每组各8只.参黄散组POI造模后参黄散外敷大鼠的脾俞和肾俞,而假手术组(仅行剖腹探查术)和模型组造模后使用等量细沙包外敷脾俞和肾俞.各组于造模后3d观察各组大鼠单位体重增加百分数、标准化摄入量、大鼠结肠传输时间、血浆激素(肥胖抑制素、胃动素、血管活性肠肽和降钙素基因相关肽)水平及各组大鼠胃体平滑肌层肥胖抑制素和GPR39受体表达.结果 造模术后3d,与假手术组比较模型组大鼠单位体重增加百分数、标准化摄入量、血清胃动素、血管活性肠肽和降钙素基因相关肽水平均下降,而血清肥胖抑制素浓度、结肠传输时间和胃体平滑肌肥胖抑制素和GPR39表达均增加(P<0.05,P<0.01);参黄散组较模型组血清胃动素、血管活性肠肽和降钙素基因相关肽水平均增加,而血清肥胖抑制素水平、结肠传输时间和胃体平滑肌肥胖抑制素和GPR39表达均降低(P<0.05,P<0.01).结论 参黄散具有显著促进术后肠动力障碍大鼠胃结肠传输功能,其作用机制可能与促进血清肥胖抑制素、胃动素、血管活性肠肽和降钙素基因相关肽分泌和降低胃体肥胖抑制素和GPR39表达有关.%Objective To investigate the effect of Shenhuang powder(SHP)on colonic transit and serum gastrointestinal hormone in rat model of postoperative ileus(POI).Methods SD rats were randomly divided into sham operation group(SHAM),intestinal manipulation group (IM)and Shenhuang powder group(SHP).The POI models were established in IM and SHP. The SHAM was operated with exploratory laparotomy only. After the model was successfuly established,SHP were transdermaly administered with SHP on the animals'Pishu points and Shenshu points while SHAM and IM with equal volume of fine sandbag. Serum level of motilin(MTL),vasoactive intestinal peptide(VIP),obestatin and calcitonin gene related peptide(CGRP) and gastrointestinal transit time were determined from each group at3d postsurgery respectively. Meanwhile,the expression of obestatin and GPR39 receptors in gastric muscularis were examined by Western blot at3d after surgery.Results At3d after surgery, the serum levels of MTL,VIP,CGRP and body weight gain and food intake in IM were dramaticaly lower,and serum obestatin level and colonic transit time and the expression of obestatin and GPR39 receptors in gastric muscularis were higher than those in SHAM(P<0.05,P<0.01). Serum levels of MTL,VIP,CGRP and body weight gain and food intake were dramaticaly increased significantly in SHP,compared with the IM and the obestatin level and colonic transit time and the expression of obestatin and GPR39 receptors in gastric muscularis were also lower than those in IM(P<0.05,P<0.01). Conclusion SHP can promote the recuperation of enterokinesia in rat model of POI through increasing the levels of MTL,VIP,CGRP and body weight gain and food intake and reducing the serum obestatin level and the expressions of obestatin and GPR39 receptors in gastric muscularis.
