髓内钉内固定与单臂外固定支架外固定加植骨治疗肱骨干骨折不愈合的疗效分析

摘要

objective Intramedullary nail fixation are discussed with single arm outer fixation bracket external fixation and bone graft in the treatment of humeral fracture bone don't heal curative effect. Methods Retrospective analysis of 46 cases of humeral fracture healing in patients with clinical data, A group of 23 cases with intramedullary nail fixation and bone graft surgery, B group of 23 patients with single arm outer fixation bracket external fixation and bone graft surgery, compared two groups of patients with fracture healing time, complications. Asaresult All patients received 12~24 months follow-up, A group of 22 patients with fracture healing, healing rate was 95.7%;Fracture healing time was 6.8 months on average, 1 case of patients with iatrogenic radial nerve injuries, recovery of five months. Group B all at the end of the follow-up of patients with fracture healing, healing rate was 100%;Fracture healing time, on average, 6 months, 1 cases of patients with postoperative infection, wound healing after application of antibiotic treatment. Conclusion For humeral fractures in patients with postoperative bone nonunion, application of intramedullary nail internal fixation with single arm external fixation bracket external fixation and bone graft treatment can obtain satisfactory clinical effect.%　　目的探讨分析髓内钉内固定与单臂外固定支架外固定加植骨治疗肱骨干骨折术后骨不愈合的临床疗效。方法回顾性分析46例肱骨干骨折不愈合患者的临床资料，将其随机分为两组，A组23例患者行髓内钉内固定加植骨术， B组23例患者行单臂外固定支架外固定加植骨术，比较两组患者骨折愈合的时间及并发症。结果所有患者均获12个月～24个月随访，A组22例患者骨折获愈合，愈合率为95.7%；骨折愈合时间平均6.8个月，1例患者出现医源性桡神经损伤，术后5个月恢复。B组患者随访结束时骨折全获愈合，愈合率为100%；骨折愈合时间平均6个月，1例患者出现术后感染，应用抗生素治疗后伤口愈合。结论对于肱骨干骨折术后骨不愈合的患者，应用髓内钉内固定与单臂外固定支架外固定加植骨治疗均可获得满意的临床疗效。
