无创正压通气治疗 AECOPD 并早期肺性脑病的临床分析

摘要

Objective To investigate the noninvasive positive pressure ventilation (NPPV) on acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and the clinical curative effect of early in patients with pulmonary encephalopathy. Methods retrospective analysis of the clinical data of 30 cases of 2012 January to June 2009 noninvasive ventilation AECOPD and early pulmonary encephalopathy in our hospital respiratory department of internal medicine patients, changes of blood gas analysis after 2h, 24h and clinical changes were observed before and after treatment.Results in this group, 30 patients in 3 cases of invasive ventilation therapy, 2 cases gave up treatment, 25 cases (83%) blood gas analysis results are improved obviously, arterial partial pressure of carbon dioxide levels decreased (P < 0.05), arterial partial pressure of oxygen levels (P < 0.05), increased pH levels (P < 0.05).Conclusion the application of BIPAP ventilator assisted ventilation in the treatment of AECOPD and the effect of early pulmonary encephalopathy is accurate, can rapidly improve AECOPD gas exchange.%目的探讨无创正压通气(NPPV)对慢性阻塞性肺疾病急性加重期(AECOPD)并早期肺性脑病患者的临床疗效。方法回顾性分析2012年1月至年6月在我院呼吸内科行无创通气的 AECOPD 并早期肺性脑病的30例患者的临床资料，观察治疗前及治疗后2h、24h 后的血气分析变化及临床变化。结果本组30例患者中3例改行有创通气治疗，2例放弃治疗，25例（83%）血气分析结果明显改善，动脉血二氧化碳分压水平降低(P ＜0.05)，动脉血氧分压水平升高(P＜0.05)，pH 值升高(P ＜0.05)。结论应用 BIPAP 呼吸机辅助通气治疗 AECOPD 并早期肺性脑病疗效确切，能迅速改善 AECOPD 的气体交换。
