ABSTRACT:Objective to evaluate laparoscopic hepatectomy (lH) versus open hepatectomy (oH) in the treatment of hepato-cellular carcinoma.Methods sixty-six hepatocellular carcinoma patients were enrolled and grouped into lH group(30 cases)ver-sus oH group (38 cases) in this retrospective study. the average operation time, fiatus time and intraoperative blood loss, mean postoperative peritoneal drainage time, mean postoperative hospital stay, Perioperative liver function function in two groups of patients were analyzed statistically. three-years survival rates and tumor recurrence rates were compared.Results no death oc-curred during the hospitalization period in this study. the operation time of lH group was longer than that of oH group, while intraoperative blood loss, the mean time of fiatus, drainage tube indwelling and the mean time of hospital stay of lH group were all significantly lower than those of oH group(P<0.05). liver function of patients in lH group was significantly higher than oH group. the difference was statistically signifibetween the two groups (P<0.05).there were no statistic difference between lH group and oH group in three-years tumor recurrence rates and survival rates(P>0.05).Conclusion lH is relatively safe and effective to help restore the health of patients with hepatocellular carcinoma compared with oH, with no differences in patients long term survival. it should be worthy of promotion.%目的：分析比较腹腔镜肝切除术与开腹肝切除术治疗原发性肝癌的临床疗效。方法回顾性分析我院2009年1月到2012年1月收治的66例原发性肝癌患者，其中28例行腹腔镜肝切除术（lH组），38例行开腹肝切除术（oH组）。对两组患者围手术期情况（术中平均出血量、平均手术时间、术后平均肛门排气时间、腹腔引流时间、住院时间、围手术期肝功能等指标）进行统计学分析。比较两组患者术后三年肿瘤复发率及生存率。结果 lH组的平均手术时间高于oH组，但术中平均出血量、术后平均肛门排气时间、腹腔引流时间和住院时间明显低于oH组（P<0.05）。两组患者的术后3年肿瘤复发率及生存率均无明显差异（P>0.05）。lH组患者术后肝功能恢复明显快于oH组，差异有统计学意义（P<0.05）。结论 lH较传统的oH治疗肝癌效果相对安全有效，可明显改善肝癌患者的围手术期状况，且远期疗效无明显差异，值得在临床开展推广。
