消糖降压汤对糖尿病合并高血压患者血清Betatrophin、AngⅡ及β细胞功能的影响

摘要

目的:探讨消糖降压汤对糖尿病合并高血压患者血清betatrophin、血管紧张素Ⅱ(Angiotensin Ⅱ,AngⅡ)及β细胞功能的影响.方法:选取2016年4月至2016年10月武汉大学人民医院收治的糖尿病合并高血压患者136例,按随机数表法分为对照组与观察组,每组68例.对照组患者采用常规西药治疗,观察组患者在对照组基础上采用消糖降压汤治疗.比较2组治疗前后临床症状、糖化血红蛋白(Glycosylated Hemoglobin,HbA1c)、餐后2 h血糖(2 Hours Postprandial Blood Glucose,2 hPG)、空腹血糖(Fasting Plasma Glucose,FBG)、收缩压(Systolic Blood Pressure,SBP)、舒张压(Diastolic Blood Pressure,DBP)、血清betatrophin、AngⅡ、空腹胰岛素(Fasting Insulin,FINS)、胰岛B细胞功能指数(Index of B-cell Function in HOMA,HOMA-β)的变化情况,并评价2组临床疗效及安全性.结果:较治疗前,2组中医证候积分、HbA1c、2 hPG、FBG、SBP、DBP、AngⅡ、FINS在治疗后均降低,而血清betatrophin、HOMA-β上升,且观察组上述观察指标均优于对照组,差异有统计学意义(P<0.05);观察组总有效率为91.18%(62/68),高于对照组的70.59%(48/68),差异有统计学意义(P<0.05);2组均未出现不良反应及肝肾功能异常.结论:采用消糖降压汤治疗糖尿病合并高血压,可显著提高临床疗效,缓解临床症状,提高血清betatrophin,降低AngⅡ,改善B细胞功能,促进患者身体恢复.%Objective:To investigate the effects of Xiaotang Jiangya decoction on serum betatrophin,angiotensin Ⅱ(AngⅡ),β cell function in patients with diabetes combined with hypertension.Methods:A total of 136 patients with diabetes combined with hypertension in our hospital from April 2016 to October 2016 were selected as research object,and they were divided into control group and research group by random number table,with 68 cases in each.Control group took conventional medicine,and research group took Xiaotang Jiangya decoction based on control group.The changes of clinical symptoms,blood glucose(glycosylated hemo-globin(HbA1c),2 hours postprandial blood glucose(2 hPG),fasting plasma glucose(FBG)),blood pressure(systolic blood pressure(SBP),diastolic blood pressure(DBP)),serum betatrophin,AngⅡ,βcell function(fasting insulin(FINS),index of β-cell function in HOMA(HOMA-β))of two groups were compared,and the clinical efficacy and safety of two groups were evalua-ted.Results:Compared with before treatment,the TCM symptom score,HbA1c,2 hPG,FBG,SBP,DBP,AngⅡ,FINS of two groups decreased after treatment,but serum betatrophin,HOMA-βincreased,and above indexes of research group were better than those of control group(P<0.05);The total effective rate of research group was 91.18%(62/68),which was higher than 70.59%(48/68)of control group(P<0.05); There was no adverse reactions and liver-kidney dysfunction in two groups.Conclusion:Xiao-tang Jiangya decoction for patients with diabetes combined with hypertension can significantly increase clinical efficacy,relieve clin-ical symptoms,increase serum betatrophin,decrease AngⅡ,improve βcell function and promote patients'recovery,which has sig-nificance of clinical promotion.
