腹腔镜胃癌根治手术中的团队配合

摘要

Objective: To study the laparoscopic gastric cancer radical surgery in the team and the importance of the approach. Methods:To collect the hospital since May 2012 to April 2014 gastric cancer patients, 30 cases, all in laparoscopic gastric cancer radical surgery, the surgeon and hand mirror aide, the first assistant in the corresponding areas are the basic fixed cooperate mode, complete the gastrocolic ligament, hepatic ligaments of the stomach and spleen and stomach ligament was surgery, for groups of lymph node cleaning, outside the body cavity to remove lesions, the digestive tract reconstruction. Results:All patients were successfully completed surgery, surgery time 289~382 min, the average operation time was (345.8 ±24.3)min;Surgical blood loss of 60~340 ml, average blood loss(154.6±34.8)ml;Cleaning the lymph node 15-32, average (23.1±3.2);Postoperative exhaust time 24~72 h, the average time (38.5±4.8)h; All patients were followed up for 1 ~ 12 months, no recurrence and metastasis. Conclusion: Closely cooperate in laparoscopic gastric cancer radical surgery can effectively shorten the operation time, less intraoperative blood loss, ensure the postoperative patients recovering well, to the effect of the surgery is of great significance.%目的：探讨腹腔镜胃癌根治手术中团队配合的重要性以及方式方法。方法：收集我院自2012年5月至2014年4月收治的胃癌患者30例，全部进行腹腔镜胃癌根治术，手术中，主刀医生和扶镜助手、第一助手在相应的区域采用基本固定的配合模式，完成对胃结肠韧带、肝胃韧带和脾胃韧带的离断手术，对各组淋巴结进行清扫，在体腔外切除病灶，重建消化道。结果：所有患者均顺利完成手术，手术时间289～382min，平均手术时间为（345.8±24.3）min；手术出血量60～340ml，平均出血量（154.6±34.8）ml；清扫淋巴结15～32枚，平均（23.1±3.2）枚；术后排气时间24～72h，平均时间（38.5±4.8）h；所有患者术后随访1～12月，均未见复发和转移。结论：在腹腔镜胃癌根治手术中严密的配合可以有效地缩短手术时间，减少术中出血量，保证了术后患者的良好恢复，对手术的效果具有重要意义。
