两种翻瓣方式在下颌中低位阻生智齿拔除中的疗效研究

摘要

目的 比较龈沟内切口袋形瓣与传统角形瓣在拔除下颌中低位阻生智齿中的临床疗效.方法 将120例中低位下颌阻生智齿患者随机分为观察组及对照组,每组60例.观察组采用龈沟内切口袋形翻瓣,对照组采用传统角形瓣.比较2组手术时间、术后并发症(包括出血、疼痛、开口受限、面颊部肿胀)的发生情况,采用SPSS 21.0软件包对数据进行统计学分析.结果 观察组和对照组手术时间分别为(16.12±1.18)min及(15.56±1.12)min,无明显统计学差异(P>0.05).观察组术后开口受限程度、面颊部肿胀发生率明显低于对照组(P<0.05).结论 龈沟内切口袋形翻瓣用于拔除下颌中低位阻生智齿时可以减轻患者术后肿胀、开口受限并发症,适宜临床推广应用.%Objective To compare the clinical effects of paramarginal flap and angular flap of gingiva in the extraction of impacted mandibular third molar.Methods One hundred and twenty patients with impacted mandibular third molar were equally divided into two groups.Patients in the experimental group were treated with paramarginal flap of gingiva,while patients in the control group were treated with traditional angular flap of gingiva.The operation time,postoperative complications including bleeding,pain and swelling,limitation of mouth opening,were observed and compared between the two groups.The data were analyzed with SPSS21.0 software package.Results The operation time was(16.12±1.18)min in the experimental group,which had no significance compared to the control group,of which the operation time was(15.56±1.12)min(P>0.05).Significant difference was found in the incidence of bleeding,degree of swelling and mouth opening after surgery.The incidence of moderate or severe pain after surgery was significantly lower in the experimental than in the control group(P<0.05).Conclusion Paramarginal flap of gingiva incision is better than tradi-tional method,with less postoperative complications,which should be widely applied in clinic.
