口服普萘洛尔及外用噻吗洛尔治疗婴幼儿血管瘤

摘要

目的 研究口服普萘洛尔及外用噻吗洛尔单用或联合治疗不同类型婴幼儿血管瘤(infantile hemangiomas,IH)的有效性及安全性.方法 320例IH患儿分别采用口服普萘洛尔(1mg/kg·d),外用噻吗洛尔(1滴,2次/d,外用)及两种方法联合治疗,治疗前均行相关检查排除服药禁忌证.分为口服治疗组(n=110)、外用治疗组(n=120)及联合治疗组(n=90),用药期间每月复查一次心电图及血常规,必要时行肺功能检查,用药时间3~12个月.结果 单用口服普萘洛尔及单用外用噻吗洛尔对IH均有一定的疗效,单用口服普萘洛尔对深部型IH效果更好(P<0.05),单用外用噻吗洛尔对浅表性IH效果更好(P<0.05),联合治疗组患儿治疗时间明显缩短(P<0.05),不良反应明显降低(P<0.05).结论 深部型IH可采用口服普萘洛尔治疗,浅表性IH可采用外用噻吗洛尔治疗,混合型IH可采用联合治疗.%Objective To research the efficacy and safety of oral propranolol and topical timolol treatment in different types of infantile hemangioma(infantile hemangiomas,IH). Methods 320 cases of IH kids were treated with oral propranolol (1mg/kg/d),topical timolol(1 drops of timolol)and combining two methods,and the contraindications were excluded by relevant checks before treatment. The patients were divided into oral treatment group(n=110),topical treatment Group(n=120)and com-bined treatment group(n=90). During the period of medication about 3 to 5 months,the electrocardiogram and blood routine were reviewed every month,and the pulmonary function was checked. Results Oral propranolol and topical timolol alone have certain effect on IH. The effect of oral propranolol on deep type IH was better(P<0. 05). The effect of topical timolol on superficial IH was better(P<0. 05). The treatment time of combined treatment group was shortened(P<0. 05). Adverse reactions were signifi-cantly reduced(P<0. 05). Conclusion The deep type IH can be treated by oral propranolol,superficial IH can be treated with topical timolol,and mixed type IH can be treated by combined treatment.
