针刺安神六穴配合四关穴治疗偏头痛的临床研究

摘要

目的：观察针刺安神六穴配合四关穴治疗偏头痛的临床疗效。方法将128例符合纳入标准的偏头痛患者随机分为治疗组和对照组,每组64例。治疗组采用针刺安神六穴配合四关穴治疗,对照组采用常规针刺治疗。观察并比较两组治疗前后及随访时疼痛数字评分法(NRS)评分、头痛发作次数、头痛持续时间、头痛程度、伴随症状、国际抑郁自评量表(SDS)评分及国际焦虑自评量表(SAS)评分的变化情况。结果两组治疗2、4星期后及随访时NRS评分及各项头痛症状及功能评分与同组治疗前比较,差异均具有统计学意义(P＜0.01,P＜0.05)。治疗组治疗4星期后及随访时NRS评分、头痛持续时间及头痛程度与对照组比较,差异均具有统计学意义(P＜0.05,P＜0.01)。治疗组治疗2、4星期后及随访时SDS评分与SAS评分与同组治疗前比较,差异均具有统计学意义(P＜0.01)。对照组治疗4星期后SDS评分和SAS评分与同组治疗前比较,差异均具有统计学意义(P＜0.01)。两组治疗2、4星期后及随访时 SDS 评分和 SAS 评分比较,差异均具有统计学意义(P＜0.05, P＜0.01)。治疗组随访时SDS评分和SAS评分与同组治疗2星期后比较,差异均具有统计学意义(P＜0.01)。结论针刺安神六穴配合四关穴是一种治疗偏头痛的有效方法,能减轻患者头痛程度,改善焦虑和抑郁情况。%Objective To observe the clinical efficacy of acupuncture at the six mind-calming points plus the four gate points in treating migraine.Method Totally 128 eligible migraine patients were randomized into a treatment group and a control group, 64 cases in each group. The treatment group was intervened by acupuncture at the six mind-calming points plus the four gate points, while the control group was by ordinary acupuncture. The Numerical Rating Scale (NRS), headache attack frequency, headache duration, headache intensity, complicated symptoms, Self-rating Depression Scale (SDS), and Self-rating Anxiety Scale (SAS) were observed before and after treatment and in the follow-up study.Result The NRS scores and the headache symptoms and functions scores were significantly changed respectively after 2-week and 4-week treatment in both groups (P<0.01,P<0.05). The NRS score, headache duration, and headache intensity in the treatment group were significantly different from that in the control group after 4-week treatment and in the follow-up study (P<0.05,P<0.01). The SDS and SAS scores were significantly changed after 2-week and 4-week treatments as well as in the follow-up study in the treatment group (P<0.01). The SDS and SAS scores were significantly changed after 4-week treatment in the control group (P<0.01). There were significant differences in comparing the SDS and SAS scores between the two groups after 2-week and 4-week treatment as well as in the follow-up study (P<0.05,P<0.01). In the treatment group, the SDS and SAS scores in the follow-up study were significantly different from that after 2-week treatment (P<0.01).Conclusion Acupuncture at the six mind-calming points plus the four gate points is an effective method in treating migraine, and it can reduce headache intensity, and improve the anxiety and depression state.
