先天性心脏病并肺动脉高压患者肺组织eNOS及Ⅰ、Ⅲ型胶原蛋白的表达和关系

Objective To study the relationship between eNOS and collagen I , Ⅲ expression in congenital heart disease (CHD) with pulmonary hypertension (PH). Methods Sixty patients with CHD were observed of pulmonary artery morphology and divided into4 groups according to Heath-Edwards pulmonary vessel pathologic grading: control group (n = 14), group I (grade I , n = 18), group II (grade II , n = 10), and group II (grade M , n = 18). Immunohistochemi-cal SP was employed to detect the expression of collagen I , collagen Ⅲ and eNOS protein in the lung tissue of these patients. Results Compared with the control group, the expression levels of collagen I and Ⅲ were significantly increased in group II , Ⅲ, the expression levels of eNOS protein were significantly decreased in group I , H , HI (P <0.01). There was a positive correlation between the expression level of collagen I , Ⅲ and the grade of PH ( P < 0.01). There was a negative correlation between the expression levels of eNOS protein and the grade of PH ( P < 0.01). There was a negative correlation between the expression levels of eNOS protein and the collagen I and DI ( P < 0.05). Conclusion eNOS may play a role in the pathogenesis of pulmonary vascular remodeling.%目的 探讨先天性心脏病(CHD)并发肺动脉高压(PH)患者肺组织中内皮型一氧化氮合酶(eNOS)与Ⅰ型和Ⅲ型胶原蛋白的表达情况及相互关系.方法 光镜下观察60例CHD患者肺小动脉形态,按照Heath-Edwards PH分级法分组:CHD无PH患者14例作为对照组,CHD合并PH患者46例为实验组,后者又分为Ⅰ、Ⅱ和Ⅲ级组.采用免疫组化SP法测定患者肺组织中eNOS和Ⅰ、Ⅲ型胶原蛋白的表达.结果 与对照组比较,Ⅱ、Ⅲ级组Ⅰ、Ⅲ型胶原蛋白表达升高(P均＜0.01),eNOS蛋白表达降低(P＜0.01).Ⅰ、Ⅲ型胶原蛋白表达与PH分级呈正相关(P均＜0.01),eNOS蛋白表达与PH分级呈负相关(P＜0.01).eNOS蛋白与Ⅰ、Ⅲ型胶原蛋白间存在明显负相关(P＜0.05).结论 eNOS可能参与CHD合并PH的肺血管重构过程.
