空腹血糖水平对缺血性脑卒中患者平均血小板体积的影响及意义

             

摘要

目的:观察空腹血糖( FPG)水平对缺血性脑卒中( IS)患者平均血小板体积( MPV)的影响,并探讨其意义。方法选取IS患者522例(IS组)、Ⅱ型糖尿病患者115例(T2DM组)、健康体检者120例(对照组)。根据FPG水平将IS组分为4组:A组(3.89 mmol/L≤FPG<5.0 mmol/L)222例,B组(5.0 mmol/L≤FPG <5.5 mmol/L)105例,C组(5.5 mmol/L≤FPG<6.1 mmol/L)115例,D组(6.1 mmol/L≤FPG<7.0 mmol/L)80例;A、B组为正常FPG组,C、D组为空腹血糖受损( IFG)组( C组为轻度IFG、D组为重度IFG)。检测各组FPG、血脂、肝肾功能、MPV,评价MPV与FPG、血脂、肝肾功能的关系。结果 A、B、C、D、T2DM组及对照组MPV分别为(8.55±0.72)、(8.88±0.86)、(8.92±0.90)、(9.12±0.96)、(12.23±1.36)、(8.63±0.72) fL;T2DM组均高于其他各组,P均<0.01;C、D组均高于A、B组,D组增高更为显著,P均<0.05。正常FPG组、IFG组、T2DM组FPG、年龄、UA、LDL-C与MPV均呈正相关(P均<0.05)。多元线性回归分析显示,各组MPV与FPG均具有独立相关性(P均<0.05)。结论 IS患者MPV随FPG升高而升高,MPV、血糖变化对其合并心脑血管疾病具有监测及预防作用。%Objective To observe the influence of fasting plasma glucose ( FPG) level on mean platelet volume (MPV) in patients with ischemic stroke (IS) and to explore its clinical significance .Methods A total of 522 patients with IS (IS group), 115 patients with type 2 diabetes (T2DM group) and 120 normal healthy persons (control group) were enrolled into this study .According to FPG levels , IS patients were divided into four groups: group A including 222 patients (3.89 mmol/L≤FPG<5.0 mmol/L), group B including 105 patients (5.0 mmol/L≤FPG<5.5 mmol/L), group C including 115 patients (5.5 mmol/L≤FPG<6.1 mmol/L) and group D including 80 patients (6.1 mmol/L≤FPG<7.0 mmol/L).Group A and B were the normal FPG group, group C and D were impaired fasting glucose (IFG) ( group C was mild IFG , group D was severe IFG ) .The parameters including FPG , blood lipids , liver and kidney func-tion, and MPV were obtained.Meanwhile, the correlations between MPV and any one of the above parameters were evalua -ted.Results MPV was (8.55 ±0.72), (8.88 ±0.86), (8.92 ±0.90), (9.12 ±0.96), (12.23 ±1.36) and (8.63 ±0.72) fL in the groups A, B, C, D, T2DM and control group, respectively.MPV of the T2DM group was significantly higher than that in any other groups ( all P<0.01).MPV of the groups C and D were higher than that in groups A and B . Moreover, MPV of group D increased more significantly (all P<0.05).MPV in the normal FPG group, IFG group and T2DM group was positively correlated with FPG , age, UA and LDL-C ( P<0.05).Multiple linear regression analysis showed that MPV in each group was independently correlated with FPG (all P<0.05).Conclusion MPV is increased in the IS patients with the increase of FPG , and it is of great significance for the surveillance and prevention of cardiovascular and cerebrovascular disease to pay close attention to MPV and blood glucose changes .
