血清中IgA/C3检测在IgA肾病诊断中的意义

摘要

Objective:To investigate the value of serum Ig A/C3 ratio in the diagnosis of Ig A nephropathy (Ig AN) .Methods :One hundred and two patients with Ig AN ,One hundred and twenty-two with other glomerular diseases (GN) ,and fifty-three healthy control subjects without kidney disease were tested for serum Ig A and C 3 levels ,and the Ig A/C3 ratio was calculated .The ROC curve was used to assess the value of serum Ig A and Ig A/C3 ratio in predicting Ig A nephropathy .Results:Patients with Ig AN had an elevated serum Ig A/C3 ratio than those with GN and the control subjects ,with an area under the ROC curve of 0 .668 ,serum Ig A/C3 ratio was not found to significantly correlate with the occur of Ig AN .However ,when serum Ig A/C3 ratio ≥4 .5 ,the probability of Ig AN is greater than 89% and 77% of these patients are male .Conclusion :In the analyse of ROC ,when the ser-um Ig A/C3 ratio is more than 4 .5 ,it can help in the diagnosis of Ig AN ,after more than 2 .43 and it have no diag-nostic value .%目的:探讨血清IgA/C3比值在IgA肾病(IgA nephropathy,IgAN)诊断中的价值.方法:选择Ig A肾病(Ig A N)患者102例,其它非Ig A型肾小球肾炎(G N)患者122例,健康人群53例.检测患者及健康人群血清中IgA、补体C3水平,计算IgA/C3比值,并进行ROC分析.结果:IgAN患者血清IgA/C3比值明显高于GN患者及健康人群,但ROC曲线分析显示(AUC=0.668),将血清Ig A/C3用于Ig A N的诊断预测无较大价值.但当Ig A/C3比值≥4.5时,患Ig A N的概率大于89%,且77%以上为男性患者.结论:Ig A/C3比值截点不同,对Ig A N的诊断预测价值不同,以Ig A/C3比值≥2.43时为截点时,无较大预测价值,但在以Ig A/C3比值≥4.5时为截点时,具有较强的诊断意义.
