Objective:To observe the therapeutic effect and toxicity of compound matrine injection and cisplatin in the treat-ment of patients with malignant pleural effusion. Methods:64 patients with malignant pleural effusion were randomized to either test group(intrapleural administration of compound matrine injection and cisplatin) or contrastive group(intrapleural administration of cisplatin).To contrast and analyse the differences between the two groups in herapeutic effect and toxicity as well as the quality of life (QOL). Results:There were differences between the two groups in the therapeutic effect and toxicity.The test group was superior to the contrastive group(P<0.05).The main toxicities were gastrointestinal reactions and bonemarrow toxicity. Conclusion:Compound matrine injection and cisplatin can improve the therapeutic effect,attenuate the toxicity in the treatment of patients with malignant pleural effusion.%目的：探讨用复方苦参注射液联合顺铂治疗恶性胸腔积液的临床效果。方法：对2011年2月～2013年5月期间我院肿瘤科收治的64例恶性胸腔积液患者的临床资料进行回顾性研究。我们将这64例患者随机分为观察组和对照组，每组各有32例患者。我们给对照组患者使用顺铂进行治疗，给观察组患者在使用顺铂（方法与对照组患者相同）治疗的基础上，加用复方苦参注射液进行治疗。结果：观察组患者治疗的总有效率为78.13%，对照组患者治疗的总有效率为53.13%。观察组患者治疗的总有效率明显高于对照组患者，二者相比差异具有显著性（P<0.05）。观察组患者生活质量的改善率为90.63%，对照组患者生活质量的改善率为65.63%。观察组患者生活质量的改善率明显高于对照组患者，二者相比差异具有显著性（P<0.05）。在治疗过程中，两组患者中均有5例患者出现了不良反应，二者相比差异无显著性（P＞0.05）。结论：用复方苦参注射液联合顺铂治疗恶性胸腔积液可有效地提高患者的治疗效果，改善其生活质量，而且较为安全。此疗法值得在临床上推广使用。
