HR-HPV检测联合LCT在矿山妇女宫颈病变筛查中的应用

摘要

Objective:To evaluate the value and significance of the second generation hybrid capture detection(HC2)for high risk human papillomavirus( HR-HPV)and thin layer cell inspection( LCT)in screening of cervical lesions women in mine. Methods:The married female employees asked to participate in the screening of cervical lesions in Jincheng Anthracite Mining Group during September 2011 to April 2012 were invited to take part in the study. HR-HPV sample of 6 325 cases was tested by HC2,2 837 cases test sample was tested by LCT. For HC2-HPV DNA testing positive,LCT abnormal cases,col-poscopy biopsy. Results:HR-HPV infection is mainly concentrated in the 30~50 years old. HR-HPV detection specificity in HC2 was 85. 1%,description HC2 high cervical lesions detected when there is no ability to correctly rule out diseases. Ap-plications HC2 and LCT combination of two methods for testing the samples,the specificity and sensitivity of the diagnosis of cervical lesions were significantly improved. 41 cases of patients with cervical inflammation,HPV infection rate was 51. 2%, The HR-HPV infection rates of CINⅠ,CIN Ⅱ,CIN Ⅲ and invasive carcinoma were 77. 8%,92. 3%,100. 0%,100. 0%, description With the increase in the severity of cervical lesions,HPV infection has increased significantly. Conclusion:HC2 for detection of HR-HPV combined with LCT inspection is an efficient and economical cervical lesions screening programs.%目的：评价应用第二代杂交捕获检测技术（ HC2）进行高危人乳头瘤病毒（ HR-HPV）检测联合液基薄层细胞检查技术（ LCT）在矿山妇女宫颈病变筛查中的价值和意义。方法：以晋煤集团2011年9月—2012年4月参加宫颈病变筛查的矿山妇女为研究对象，HC2检测HR-HPV样本6325例，LCT检测样本2837例，对LCT异常的病例，进行阴道镜活检。结果：HR-HPV感染年龄主要集中于30~50岁；HC2检测HR-HPV特异度为85.1％，说明HC2检测在宫颈病变不存在时能正确排除疾病的能力较强；应用HC2与LCT两种方法联合对样本进行检测，对诊断宫颈病变的特异度、敏感度均显著提高；41例宫颈炎症患者中，HPV感染率为51.2％，宫颈上皮内瘤变（CIN）Ⅰ，CINⅡ，CINⅢ级和浸润癌患者中HR-HPV感染率分别为77.8％，92.3％，100％，100％，说明随着宫颈病变严重程度的增加，HPV感染率也明显增加。结论：应用HC2进行HR-HPV检测联合LCT是一种高效而经济的宫颈癌前病变筛查方案。
