目的 探讨前后路脊柱融合术治疗腰椎退行性变的临床效果.方法 应用前后路脊柱融合术治疗腰椎退行性变31例,其中腰椎间盘突出症17例,腰椎失稳、椎管狭窄14例,观察治疗效果.结果 术后29例腰痛消失,下肢麻木、疼痛逐渐好转,腰部稳定有力;2例患者术后患肢麻木感仍存在,残留有疼痛,影响行走功能.疗效评价优74.2%,良22.6%,差3.2%.术后随访腰椎间盘CT平扫,见椎体间相对运动消失,椎间隙较术前增高,椎间融合器与上下椎体融合,滑脱的椎体较术前复位明显,无移位并发症.结论 前后路脊柱融合术治疗腰椎退行性变符合脊柱的生理负重,创伤小,近期效果满意.%Objective To exptore the methods and clinical effect of anlerior and posterior spinal fusion in treatment of the de-generalion of lumbar inlervertebral disc. Methods From 2006,1 to 2011,1,17 cases of lumbar intervertebral disc hernialion and 14 cases of spinal canal stenosis were retrospectively analyzed after operalion. Results Anterior and posterior spinal fusion with decompression of lumbar spinal canal can improve the lumbar compression symptoms in the treatment of lumbar degeneralive diseases. Postop-eralive CT examination shows the disappearance of the relative molion between the vertebrae, the inlerverlebral space is increased, and shows the fusion apparatus fused with fluctuation vertebral. Besides, the reset of the displaced vertebral body is obviously,no Displacement complications were observed. Conclusions This melhod conforms to the spinal physiotogic weight-bearing with less surgery trauma. Clinical results are satisfactory.
