乳腺癌患者接受TEC方案与EC-T方案治疗后白细胞水平的变化

摘要

目的 研究乳腺癌患者接受TEC和EC-T化疗方案治疗后白细胞水平变化.方法 选取106例乳腺癌患者作为研究对象,按照随机数字表法分为2组,对照组采用多西紫杉醇(T)、表阿霉素(E)、环磷酰胺(C)联合(TEC方案)化疗治疗6周期,观察组采用序贯(EC-T方案)化疗治疗4周期.对比化疗前后2种方案对患者的白细胞水平改变及白细胞下降率的影响.结果 治疗7～14天患者白细胞水平降低最为明显,治疗的第7、14、21天2组白细胞水平均下降,但观察组略高于对照组(P<0.05);治疗的第7、14、21天观察组白细胞减少症发生率分别为18.52％、25.93％、37.03％,低于对照组的28.85％、44.23％、65.38％,差异有统计学意义,P<0.05;取4个治疗周期白细胞水平比较,观察组均略高于对照组,组间比较差异有统计学意义,P<0.05.结论 TEC治疗方案与EC-T方案相比,患者白细胞水平降低更严重,建议在病情允许的情况下选择EC-T方案化疗,以减少化疗引起的白细胞降低并发症,促进病情恢复.%Objective To study the change of leukocyte level in patients with breast cancer after treatment with TEC and EC-T chemotherapy.Methods 106 cases of breast cancer patients,according to the random number table method,were divided into 2 groups,the control group(n=52)was treated with docetaxel(T),epirubicin(E),and cyclophosphamide(C)(TEC)chemotherapy for 6 cycles,the observation group(n=54)received sequential EC-T chemotherapy for 4 cycles,change of white blood cells and white blood cells descent rates of the 2 groups before and after chemotherapy were compared.Results White blood cell in 7～14 d of treatment were significantly reduced,white blood cell levels of the 2 groups at 7 d,14 d,21 d were decreased,but the observation group was slightly higher than that of the control group(P<0.05);the incidence of leukopenia at 7 d,14 d,21 d of the observation group were respectively 18.52％and 25.93％and 37.03％,which were lower than the control group,28.85％and 44.23％,65.38％,P<0.05,the difference was significant;white blood cell level of 4 treatment cycles were compared,the observation group were slightly higher than the control group,P<0.05.Conclusion TEC regimen has more serious leukopenia than EC-T scheme,it is recommended to select EC-T chemotherapy if the condition allows,in order to reduce chemotherapy-induced leukopenia and promote recovery.
