目的：探讨腹腔镜下行腹主动脉旁淋巴结清扫术应用于子宫内膜癌治疗的临床疗效，并分析其安全性。方法选取子宫内膜癌患者90例为研究对象，应用随机数字表法分为观察组与对照组，各45例，其中观察组患者行腹腔镜下腹主动脉旁淋巴结清扫术，对照组患者行传统开腹腹主动脉旁淋巴结清扫术，对比两组患者手术情况、术后恢复情况、术中或术后并发症发生情况，并进行随访。结果与对照组比较，观察组的腹主动脉旁淋巴结切除淋巴结数增多，出血量降低及术后并发症发生率降低，术后肛门排气时间及术后住院时间均缩短，腹主动脉旁淋巴结切除时间延长，差异有统计学意义（P﹤0.05）；与对照组比较，观察组患者术后淋巴囊肿、尿潴留、切口裂开、肺动脉栓塞、深静脉血栓及腔静脉损伤的发生率降低，差异有统计学意义（P﹤0.05）。结论腹腔镜下行腹主动脉旁淋巴结清扫术应用于子宫内膜癌的治疗疗效显著，值得临床推广应用。%Objective To investigate the clinical efficacy and safety of laparoscopic para-aortic lymph node dissec-tion in the treatment of endometrial carcinoma. Method 90 patients with endometrial cancer were included in the analy-sis, and were randomized as study group or control group, with 45 cases in each, and the study group was administered with laparoscopic para-aortic lymph node dissection, while the control group underwent conventional open surgery for pa-ra-aortic lymph node dissection, and then the surgery indexes, postoperative recovery, intraoperative or postoperative complications of the two groups were observed and compared, as patients were followed. Result Compared with the control group, the study group had more dissected lymph nodes, less blood loss and lower postoperative morbidity rate, shorter time to anal flatus and less postoperative hospital stay, as well as prolonged time to paraaortic lymph node resec-tion, and all differences were statistically significant (P<0.05);Besides, the study group had lower incidence of postopera-tive lymph cyst, urinary retention, wound dehiscence, pulmonary embolism, deep vein thrombosis and vena cava injury, though the difference was of no statistical significance (P>0.05). Conclusion Prominent therapeutic effect was observed for laparoscopic para-aortic lymph node dissection as applied in the treatment of endometrial carcinoma, and it is worthy of clinical application.
